Greater Health Now recognizes that health and health care are local, and that a collaborative community approach is necessary to achieve Washington’s aims of better health, better care, and lower costs. No single sector or organization in a community can create transformative, lasting change in health and health care alone. Clinical, community and government entities must coordinate their efforts and actions around clearly defined goals that support whole-person health.
Representing nine counties and a tribal nation, Greater Health Now works with seven Local Health Improvement Networks (LHINs) to identify the Social Determinants of Health (SDOH) specific to their area. Once the most critical SDOH are determined, Greater Health Now provides funding support to philanthropy organizations to align services to address those areas of need. Third-Party administrators receive a portion of funds to address these SDOH selected by the LHIN they serve and disperse the funding across their county.
At our upcoming October Leadership Council meeting, Greater Health Now has asked LHIN leaders to share any success stories, upcoming projects, or activities they would like to share from 2022. Each LHIN will be given 15 minutes to share these reports during our meeting.
The virtual October Leadership Council is set for Thursday, October 20, 2020, from 9:00, am - 12:30 pm.
