Care Connect Program

Have You, or Someone Your Organization Serves, Been Impacted by Covid-19? We Can Help.  

The Greater Health Now Community Hub delivers the Care Connect Washington program in the GHN region (Asotin, Benton, Columbia, Franklin, Garfield, Kittitas, Walla Walla, Whitman, Yakima Counties, and the Yakama Nation.) Through this innovative program, care coordinators connect people to a wide range of community-based services they are eligible for, such as medication delivery, health care, help applying for unemployment, local housing agencies, food banks, childcare providers, and more.  

The Care Connect program is open to all residents within the GHN region (Asotin, Benton, Columbia, Franklin, Garfield, Kittitas, Walla Walla, Whitman, Yakima Counties, and the Yakama Nation) who have been impacted by the COVID-19 pandemic and coordination is free of charge. Benefits include service coordination, health education, coaching, goal setting, action planning, and system navigation. Care Coordinators from our Care Coordination Agencies work with clients over three to six months and check in regularly to help clients achieve their goals. Information is provided to clients in their preferred language. 

How to Refer a Client

Community-based organizations and agencies providing health and social services can use Greater Health Now’s online intake form to refer clients directly to the program. A Care Coordinator will contact the client to follow up on the referral within three business days. Go to Referrals Page HERE.

How to Self Refer

Community members can call 1-833-453-0336 to complete an intake for the program. Language assistance is available.

Changes to the Program

As of July 1, 2023, the Care Connect Washington program has changed. Instead of providing immediate support and direct services to people who test positive for COVID-19, the program will now offer care coordination and connection to services for people trying to recover from the impacts of COVID. As of this date, direct services (grocery delivery, care kits, non-perishable food kits, and household financial assistance) are no longer available for Care Connect Washington clients. Are Care Connect Washington Services available to me?

You’re eligible for services if you’ve been affected by COVID-19. That includes people who are currently sick with COVID-19 or had COVID-19 in the past.

A local care coordinator will contact you to understand your needs and help you connect to resources and services.

What kind of assistance can I expect?

Your local care coordinator will spend time getting to know you and understanding your needs. Then they will identify local resources available to support you, including health care, subsidized housing or child care, unemployment benefits, food assistance, and more. Services will vary by region. Care Connect Washington will provide the information you need in your preferred language. Care Connect services are available at no cost.

How long will it take before I receive assistance?

Your regional care coordinator will make every effort to provide assistance within one day of receiving a referral.

How will you protect my information?

Your care coordinator will confirm the information they get from the call center and ask about your needs. Any information you share with your care coordinator remains confidential. The employees who provide care coordination services and the Department of Health will use the information to identify essential services that you may need. They will not share personal or private health information.
