How to Make Referrals
Connecting People to Community-Based Care
How it Works:
It can be hard for people to access the services and resources they need to thrive. The Greater Health Now Community Hub matches individuals and families in the Greater Health Now region with community-based workers who help them find resources, navigate systems, and champion their needs. Referrals for services can be made by an individual, care provider, or family member. If you are looking to refer a potential client to the Greater Health Now Community Hub, click the button below to use the online intake form or call the Community-Based Care Hub Referral Line: 509.816.4460. Greater Health Now does not provide direct health care services.
The online intake form is a secure and safe way to connect clients with the Community Hub.
