The Greater Health Now Community Hub matches individuals with culturally and linguistically appropriate Care Coordinators from our Hub network to provide Care Connect Washington services. Eligibility is open to all residents within the GHN area who have been impacted by the COVID-19 pandemic and are looking for care coordination support.
