Greater Health Now partners with many different organizations throughout our nine counties to improve the health outcomes. We partner with these organizations in a few different ways.
A Social Care Network connects people to non‑clinical essentials that support health like safe housing, nutritious food, transportation, and family support. Through trusted community partners and one‑on‑one care coordination, it helps people access the resources they need to stay healthy.
Mobile Integrated Healthcare (MIH) is a coordinated, patient-centered, evidence-based, holistic model of care using collaborative, interdisciplinary teams to serve patient needs at the most appropriate level of care at a safe location of their convenience.
Enhanced Primary Care (EPC) is a comprehensive strategy to strengthen and support primary care through aligned payment methodologies, increased investment, and care delivery transformation.
Washington Health Benefits Exchange (WAHBE) is a public-private partnership established by the Washington State Legislature in 2011 with a mission to radically improve how Washington residents access health and dental coverage.
